The Grosvenor Practice offers a wide range of medical forms, reports and certificates where clinically appropriate.
This may include DVLA medicals, taxi and bus driver medicals, shotgun and firearms certificates, adoption medicals, insurance reports, return-to-work notes, fit-to-fly certificates and sick notes, as well as other formal medical reports and certificates.
We can advise on and complete many common medical forms and certificates, grouped into four broad areas.

Some forms and certificates require specific training, statutory approval, occupational health accreditation, specialist assessment or access to full medical records.
We may not be able to complete certain reports — including some visa medicals, specialist occupational certificates, or documents requiring certification by a particular approved doctor.
Where the form can be completed within a 30-minute appointment, this is included in the appointment fee.
Some reports require additional work after the appointment — such as reviewing lengthy paperwork or accessing records. Any extra fees will be discussed with you in advance.
